JOB SUMMARY:

• To work as a member of the community nursing team to provide high quality, evidence-based, holistic nursing care to patients within the primary care setting.
• To assess, plan, implement and evaluate packages of care based upon individual patient needs, under the guidance of the Community Nursing Team Leader
• To undertake managerial responsibility for the community nursing team in the absence of the Team Leader

LIAISES WITH:

• General Practitioners, Social Workers, Families, Carers, Multi Disciplinary Team, Third Sector Organisations

KEY RESPONSIBILITIES:

Clinical
1. Assess, plan and evaluate individual packages of care under the guidance/supervision of the Team Leader
2. Competently perform clinical skills related to the post
3. Assess health needs at individual, practice and community level
4. Understand how to use and maintain equipment safely and competently, following appropriate
policies and procedures
5. Professionally responsible for a delegated caseload under the supervision of the Team Leader
6. Contribute to the development of clinical guidelines
7. Maintain professional records in accordance with the Trust and NMC Record Keeping Policies
