RESPONSIBLE FOR:

A defined population of patients in a variety of settings across the Partnership Trust including the community and hospitals.

WORKING RELATIONSHIPS

INTERNAL: Long Term Conditions Team Specialist Nursing and Therapy colleagues, Service Managers, Clinical and Professional Leadership Team, Hospital and Community Nursing and Allied Health Professional Colleagues, Corporate teams

EXTERNAL: Acute Care Services, Commissioners, GPs and Primary Care Providers

JOB PURPOSE.

The post holder will be part of the Community Heart Failure Nursing team providing a comprehensive and holistic service for patients with a confirmed diagnosis of heart failure. The role requires the post holder to work in a variety of settings, including clinics and the patient's home.

This is a dynamic post that requires the post holder to work closely with the multi-disciplinary teams / agencies in addressing a range of complex care / issues across the spectrum of Heart Failure management, preventing unnecessary hospital admissions and improving the health and wellbeing of the population served. The nature of the service will be determined by local service specifications.

The post holder will be expected to demonstrate Trust Values at all times and demonstrate initiative and creativity towards problem solving.
