Band 3 Community Support Worker x 2 for Dementia Service

The Dementia Assessment & Support Service based at Severn Fields, Shrewsbury, are looking for 2 x Band 3 Community Support Workers to support their team. This role will require the workers to facilitate pre-assessment information gathering for those referred into service and also undertaking of ECG's, BP and Pulse.

  • Band 3 Community Support Worker x 2
  • Full time, Monday - Friday: 9am - 5pm
  • Start ASAP for 12 weeks
  • Previous experience working with dementia would be preferred
  • Blended working, some office but can be mainly home working.

RESPONSIBLE FOR:
* The post holder will be responsible for working in the Dementia service, with the flexibility of working across pathways, dependent on skill and service need.

* The post holder will be responsible for delivering planned interventions, in accordance with an agreed care plan, to an identified group of people experiencing dementia and memory related mental health difficulties.

JOB PURPOSE:

The post holder is responsible to support the qualified clinicians in the delivery of person centred care based upon the person's negotiated goals and relevant evidence based interventions. The post holder, in conjunction with the multi-disciplinary team, will support the qualified staff in providing planned interventions to support the person to meet their agreed goals and coordinate and/ or participate in the review of progress towards those goals, utilising recommended outcome measures, involving the service user and any identified carer throughout the clinical pathway.
