Medacs Healthcare is looking for experienced, qualified Community Nurse to deliver care to patients who are in their own homes or residential homes, and or delivery of care to patients within a clinical setting where these are available and appropriate.

We are looking for dedicated and compassionate nurses to further enhance the high-quality patient experience that Midlands Partnership are currently offering in the community. You will be working closely with nurses, AHPs including podiatry, physiotherapists, falls team, stroke and rehab team as well as the out of hours service to ensure that patients are at the centre of the care provided.

Main Duties/Responsibilities

As a Community Nurse with Midlands Partnership NHS Trust you will assess new service user referrals and care/support needs. The care and support plans may include;

  • Promoting independence/self management and maintaining safety
  • Improvement in health and wellbeing including mental health
  • Interventions in disease/condition management
  • Prevention and reducing of health inequalities
  • Admission avoidance
  • Early supported discharge
  • Case management/ treatment/ care plans
  • Bladder, catheter, bowel care, palliative and end of life care

Qualifications/Licenses/Certifications

Essential

  • Be a registered nurse.
  • At least 2 years post-graduate experience.
  • 6 months recent NHS experience within the last 3 years.
  • You will need to pass an enhanced background check.
