Starting the day: patient reviews and handover 

You’ll begin with a ward handover, reviewing: 

  • New patient admissions 
  • Medical history and current conditions 
  • Medicines prescribed before and during hospital stay 
  • Allergies and special requirements (e.g. blister packs) 

You’ll often speak directly with patients, or where needed, work with families or community pharmacies to build a complete and accurate medication picture.

Reviewing drug charts and preventing errors 

A key part of the role is ensuring accuracy and safety in prescribing. 

You’ll: 

  • Check drug charts against patient history 
  • Identify missing or incorrect medications 
  • Flag high-risk drugs (e.g. Parkinson’s medication) 
  • Work with doctors to resolve discrepancies 

✅ Impact: This work directly prevents medication errors and improves patient safety. 

Clinical screening and medicines optimisation 

Using systems such as Integrated Clinical Environment (ICE), you’ll assess: 

  • Blood test results 

  • Liver and kidney function 

  • Treatment effectiveness 

From this, you’ll make clinical recommendations on: 

  • Dosage adjustments 

  • Stopping or changing medicines 

  • Alternative treatments where needed 

You’ll also support patients who cannot take medicines in standard formats, advising on liquids, alternatives, or administration via feeding tubes. 

Managing medication supply on the ward 

Hospital pharmacists ensure wards have the medicines they need, when they need them. 

This includes: 

  • Ordering stock medicines 

  • Ensuring timely supply for new prescriptions 

  • Minimising missed doses

  • Correct use of devices (e.g. inhalers)
