Great opportuity for an experienced band 6 mental health Nurse to support a community mental helath team

Opportunities have arisen for Community Mental Health Nurses (Care Co-ordinator) within both East and West CMHTs. The CMHT work with people aged 16 and over with severe and enduring mental health and social care needs.

Care co-ordinators are responsible for providing holistic assessment, interventions, risk assessments and care plans for their caseload.

Main requirements for the role

Main duties

Assessment of health and social care needs

Producing, monitoring and evaluating collaborative care plans

Risk assessment and management

Providing evidence based therapeutic interventions.

Providing education advice, emotional and psychological support

To promote choice in service user involvement in care planning and risk management
