Rate: Band 6

Skills required / Qualifications needed: Adult Mental Health

Role will be to conduct initial assessments for patients referred to LMHT. Documentation of assessments, formulation of suitable ongoing plan of care. Making referrals to and liaison with other agencies. Knowledge of and able to identify any safeguarding concerns and escalate accordingly.

Hours: 37.5 per week, Monday-Friday 9-5.
