An exciting opportunity for a Senior Nurse Assessor within the continuing healthcre team at Birmingham & Sollihull for a full time, temporary position with a competitive day rates in a well supported team.

The Role

We are seeking a dedicated Continuing Healthcare Case Manager to join our Adult Assessment and Continuing Healthcare (AACHC) team. You will be responsible for:

• Conducting comprehensive Continuing Healthcare (CHC) and Funded Nursing Care (FNC) assessments for adults (18+) with complex health needs.
• Managing retrospective and appeal cases within defined timescales.
• Acting as Case Manager for individuals receiving CHC funding, ensuring care is appropriately planned, delivered, and reviewed.
• Facilitating multi-agency collaboration to ensure clients receive optimal care in the most suitable environment.
