Job Summary
We're looking for a skilled and supportive Community Mental Health Practitioner to work with adults experiencing a range of mental health needs within community mental health services. You'll complete specialist assessments, develop formulation‑based care plans and deliver brief, evidence‑informed interventions in the community.
You'll manage your own caseload, contribute to effective risk management and support recovery, independence and wellbeing through person‑centred care. This role suits a confident mental health professional who enjoys working autonomously and engaging people with complex needs.
About You
You're a Registered Mental Health Nurse who builds trust easily and works confidently in fast‑paced community settings. You value face‑to‑face work, collaborative practice and supporting people who may find it difficult to engage with services.
You bring strong assessment and formulation skills, emotional resilience and a genuine commitment to helping adults improve their quality of life.
