Reporting To: Ward Manager / Senior Sister / Charge Nurse
Job Purpose
The post holder is responsible for the assessment, planning, implementation, and evaluation of evidence-based nursing care, working collaboratively and cooperatively within a multi-disciplinary team to meet the holistic needs of patients and their families. The successful candidate will assist in the smooth running and organization of their clinical area, ensuring safe, high-quality patient-centered care while contributing to the mentorship and supervision of student nurses and healthcare support workers.
1. Clinical Practice & Patient Care
Conduct comprehensive, evidence-based patient assessments, developing and updating individualized care plans.
Deliver high-quality clinical interventions including medicine management, wound care, venepuncture, and monitoring vital signs according to BCUHB policies.
Recognize acute changes in a patient's physical or psychological condition and escalate appropriately to the medical team or shift leader.
Ensure accurate, real-time, and legally compliant record-keeping in accordance with the NMC Code of Conduct.
Adhere stringently to infection control guidelines, safeguarding procedures, and clinical governance frameworks.
2. Communication & Collaboration
Communicate compassionately and effectively with patients, relatives, and carers, providing emotional support and updates on treatment plans.
Liaise productively with the wider Multi-Disciplinary Team (MDT), including consultants, therapists, and the discharge planning team to promote smooth patient pathways.
Provide clear, structured handovers during shift changes to maintain continuity of care.

| Qualifications | * Registered General Nurse (Adult) with a valid, current UK NMC pin. * Evidence of continuing professional development (CPD). | * Mentorship/Teaching qualification. * Advanced life support or clinical specialty-specific training. |
| Experience | * Proven pre-registration or post-registration experience in a direct, acute nursing care setting. * Experience in providing holistic nursing care. | * Experience working within an acute NHS district general hospital environment. * Experience in palliative or terminal care. |
| Skills & Knowledge | * Deep understanding of clinical governance, risk assessment, and patient safety protocols. * Proficient clinical skills (cannulation, catheterization, wound care, etc.). * Competent IT skills for electronic patient records. | * Ability to speak Welsh (or a willingness to learn). * Phlebotomy skills. |
