Swansea Bay University Health Board is investing in an ambitious programme of integrated frailty care, creating an exciting opportunity for Consultant Geriatricians to join a progressive, high-performing team. Based at Morriston Hospital, this role offers the chance to influence innovative services spanning acute frailty, community care, virtual wards and a developing Hospital at Home model. Working alongside an experienced multidisciplinary team, you will help deliver comprehensive geriatric assessment across hospital and community settings while contributing to one of the most forward-thinking frailty strategies in Wales

The Role

The successful candidate will provide consultant-led care for older adults across acute, ambulatory and community pathways. Clinical responsibilities may include:

  • Leading Acute Frailty services and supporting the frailty single point of access.
  • Providing comprehensive geriatric assessment for older people living with frailty.
  • Supporting the development of Hospital at Home and community-based care pathways.
  • Managing patients within Acute Frailty Units and dedicated inpatient geriatric wards.
  • Working closely with Advanced Clinical Practitioners and multidisciplinary teams.
  • Contributing to undergraduate and postgraduate education.
  • Participating in quality improvement, service development and research initiatives.

Person Specification Essential

  • Full GMC registration with a licence to practise.
  • MRCP (UK) or equivalent specialist qualification.
  • On the GMC Specialist Register for Geriatric Medicine and General Internal Medicine, or within six months of CCT at interview.
  • Broad experience in Geriatric Medicine and General Internal Medicine.
  • Commitment to multidisciplinary working, education and service improvement.

Desirable

  • Experience in frailty pathway development.
  • Research, audit or quality improvement experience.
  • Additional teaching or medical education qualifications.
  • Sub-speciality interests relevant to older people's services.
